Part Time - Registered Nurse - Home Health

Role: The RN Home Health provides comprehensive in-home nursing care, including initial, recertification, and routine visits. They assess patient needs, develop and coordinate individualized care plans, perform nursing procedures, and collaborate with physicians, patients, families, and interdisciplinary teams to ensure optimal care delivery. Responsibilities Assess and manage patient needs following physician orders using the nursing process.Coordinate care plans with multidisciplinary teams and develop instructions for Home Health Aides.Provide patient and caregiver education to promote self-care and recovery independence.Communicate patient status, lab results, and care updates with physicians and team members.Ensure compliance with regulatory guidelines, complete documentation accurately and timely, and participate in performance improvement activities.Support staff education, participate in meetings, and contribute to departmental performance initiatives. Qualifications & Skills Licensed registered nurse with a minimum of 1 year medical-surgical, home health, or ICU experience.Strong critical care nursing skills, computer proficiency, and knowledge of healthcare documentation systems (Epic, 3M preferred).Required certifications include RN license, BLS, Georgia driver’s license, and private vehicle insurance.
