Part-Time RN Case Manager - Home Health

Overview: This role involves providing skilled home care nursing visits across multiple counties, serving patients of all ages. Key Responsibilities Coordinate and deliver direct and indirect patient care, including complex wound care, PICC lines, TPN, catheter management, and tracheostomy care.Assess, plan, implement, and evaluate patient care using the nursing process.Educate patients and families to promote independence and wellness.Manage documentation, including OASIS assessments, physician orders, and visit records, ensuring compliance with regulatory standards.Supervise Home Health Aides, assign tasks, and provide instructions.Act as a liaison within the interdisciplinary team, facilitating case conferences and communication with physicians and caregivers.Perform case management activities, including assessments, care planning, and monitoring. Qualifications & Skills Current IL RN license and BLS certification.Minimum of 1 year nursing experience.Valid driver’s license and proof of auto insurance.Knowledge of Medicare benefits, regulatory documentation standards, and complex wound management.Strong communication skills and ability to work independently with autonomy in a supportive team environment.
