Home Care RN Case Manager | Part Time

About the role

The Home Care RN at Peterson Health will perform various duties including admission of patients, recertification, resumption of care, supervisory and routine visits. They will implement the plan of care for patients, attend Case Conferences and Weekend Reports, and document OASIS data sets. The RN will provide necessary care as per departmental policies and procedures and as directed by the supervisor.

Responsibilities

• Admission of patients, recertification’s, resumption of care, supervisory and routine visits
• Implement the plan of care for patients
• Attend Case Conferences and Weekend Reports
• Perform and document OASIS data sets
• Provide care needed as described in departmental policies and procedures and as assigned by supervisor

Requirements

• Valid RN License in State of Texas
• Current American Heart Association CPR Certificate
• Valid Driver’s License and Car Insurance
• Minimal 1 year in medical setting experience
• 1-5 years home health experience preferred

Qualifications

• Required Education: Valid RN License
• PREFERRED Education: Bachelors Degree in Nursing

• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite
