Part-Time Home Health Case Manager

Key Responsibilities

• Coordinating and delivering direct and indirect patient care using the nursing process.
• Performing comprehensive assessments, wound care (including wound vacs), central line management, and other skilled nursing procedures.
• Managing care plans, educating patients and families, and ensuring documentation compliance.
• Acting as a liaison within the interdisciplinary team, communicating with physicians, caregivers, and other healthcare professionals.
• Supervising Licensed Practical Nurses and Home Health Aides, assigning tasks, and providing instructions.
• Completing OASIS assessments, ensuring timely documentation, and adhering to regulatory standards.
• Monitoring health conditions, promoting independence, and supporting patient wellness.

Qualifications And Skills Required

• Current IL RN license and BLS certification.
• College diploma in Nursing with at least 1 year of nursing experience.
• Valid driver’s license and proof of auto insurance.
• Knowledge of Medicare benefits, regulatory documentation standards, and case management activities.
• Strong communication skills, attention to detail, and ability to work independently with autonomy.
