Part-Time Home Health Case Manager

Join our team as a Home Care Registered Nurse/Field Case Manager, delivering skilled nursing visits across multiple counties. Key Responsibilities Include Coordinating and providing direct patient care through assessments, care planning, and education in areas such as wound care, PICC lines, TPN, catheters, tracheostomy, and health condition monitoring.Managing case activities including documentation, recertifications, and updates via OASIS/485 within specified timeframes.Collaborating with interdisciplinary team members, physicians, patients, and families to ensure seamless care and support.Supervising home health aides and directing their activities according to care plans.Maintaining compliance with regulatory standards, accurately documenting all care activities, and performing complex wound and central line management.
