Part-Time RN Case Manager - Home Health

About the role

The Home Care RN/Field Case Manager provides skilled nursing visits to patients of all ages across multiple counties, working independently within an interdisciplinary team.

Responsibilities

• Coordinate and deliver direct and indirect patient care, including assessments, care planning, and education.
• Perform complex wound care, manage PICC lines, ports, blood draws, TPN, antibiotics, catheters, drains, tracheostomy, and monitor health conditions to promote independence.
• Complete documentation such as OASIS assessments, physician orders, visit notes, and ensure compliance with regulatory standards.
• Act as a team coordinator, communicate with physicians and other healthcare providers, and supervise home health aides and LPNs.
• Perform case management activities, ensuring timely interventions and adherence to policies.

Qualifications & Skills

• Valid IL RN license, BLS certification, and minimum one year of nursing experience.
• Proficiency in Medicare benefits, regulatory documentation, wound care, IV access, and patient education.
• Strong communication, organizational skills, and ability to work autonomously within a team environment.
