Responsibilities

• Leading and fostering collaboration within healthcare teams
• Assessing patient needs and coordinating appropriate care levels
• Facilitating safe patient transitions and resource utilization

Qualifications

• Minimum of one year hospital-based Case Management experience or five years in healthcare
• Graduate of an accredited Registered/Professional Nursing Program
• Basic Life Support (BLS) Healthcare Provider certification upon hire
