Part-Time RN Case Manager

This role requires critical thinking, effective communication, and decisive judgment to build positive relationships and lead others. The incumbent will provide education and resources to support patient care, utilization of services, and safe patient transitions for patients, families, and healthcare teams. Must have a minimum of one year of hospital-based Case Management experience or five years in healthcare.Graduate of an accredited Nursing Program.Required Basic Life Support (BLS) Healthcare Provider certification upon hire. The position emphasizes critical thinking, effective communication, leadership, and education to ensure quality, patient-centered care and safe transitions within healthcare settings.
