Part-Time RN Case Manager

About the role

We are seeking a Registered Nurse (RN) Case Manager to lead participant care within an interdisciplinary team (IDT).

Responsibilities

• Conducting nursing assessments and developing personalized care plans
• Delivering and documenting nursing interventions, including medication administration and maintaining safe environments
• Collaborating with physicians, IDT members, participants, and families to coordinate and revise care plans
• Providing education on disease management, self-care, and prevention
• Performing routine follow-ups and nursing care in participants’ homes

Qualifications and Skills

• Graduate of an accredited nursing program; BSN preferred
• Unencumbered RN license in the state
• Current BLS/First Aid certification; CPR and First Aid certification required within 180 days if not already held
• At least two years of clinical nursing experience, preferably with elderly or frail populations
