Registered Nurse Case Manager for the PACE Home Care program.

Responsibilities

• Manage a panel of participants, conducting in-home assessments.
• Develop and coordinate individualized care plans.
• Collaborate with an interdisciplinary team to optimize health outcomes.
• Educate patients and families about care and resources.
• Ensure smooth transitions of care and connect participants with community resources.

Requirements

• Current RN licensure.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a team environment.
• Patient-centered approach with compassion and dedication.
• Experience in care coordination and assessment preferred.
