Part Time RN Care Manager

About the role

The Care Management role involves facilitating discharge planning from admission to discharge in collaboration with the healthcare team, ensuring quality patient care, safety, and efficient patient throughput.

Responsibilities

• Coordinate seamless care transitions and promote team collaboration based on metrics and best practices.
• Intervene with patients with complex or high-risk psychosocial needs as assigned.
• Perform assessments and develop care plans aligned with patient needs across all age groups.
• Effectively interact with diverse individuals and adhere to patient age-specific needs, including neonatal to geriatric care.
• Commit to quality, safety, and collaborative healthcare delivery.

Requirements

• Graduate of an accredited Nursing program with current state licensure.
• Bachelor's Degree in Nursing required.
• 2-3 years of acute care experience preferred.
• CPR certification at hire or within 90 days; case management certification preferred.

Skills

• Strong communication, assessment, time management, and critical thinking skills.
• Advanced computer proficiency.
• Knowledge of care guidelines such as InterQual or Milliman preferred.
