About the role
The Care Manager facilitates discharge planning from admission to discharge, collaborating with the healthcare team to ensure seamless patient transitions. They may intervene with patients who have complex or high-risk psychosocial needs, promoting quality patient care, safety, and efficient throughput.
Responsibilities

- Overseeing length of stay
- Supporting best practice adoption to optimize performance
Qualifications & Skills
- Graduate of an accredited Nursing School and current state license
- Bachelor's Degree in Nursing
- 2-3 years in an acute care hospital setting preferred
- Strong communication, assessment, time management, and critical thinking skills
- Advanced computer skills and knowledge of care guidelines preferred
- BLS certification required within 90 days of hire
