Part Time RN Care Manager

Role Overview: The Care Manager plays a key role in facilitating seamless discharge planning from admission to discharge, collaborating with the healthcare team to ensure quality patient care, safety, and efficient throughput. Responsibilities Coordinate discharge processes and intervene with patients having complex or high-risk psychosocial needs.Promote a coordinated care model that integrates clinical, safety, and financial components.Support performance optimization by adopting best practices and promoting team collaboration. Qualifications & Skills Graduate of an accredited School of Nursing and holds a current state license.Bachelor’s Degree of Science in Nursing required.2-3 years of experience in an acute care hospital setting preferred.Knowledge of care guidelines such as InterQual or Milliman is advantageous.Strong communication, assessment, critical thinking, and computer skills.Certification in Basic Life Support (BLS) required; Case Management certification preferred. Additional Details: Must provide patient-centered care tailored to all age groups, from neonatal to geriatric, ensuring alignment with organizational values and standards.
