Part Time RN Care Manager

About the Role

The Care Manager facilitates seamless discharge planning from admission to discharge, ensuring quality patient care, safety, and efficient throughput in collaboration with the healthcare team. They intervene with patients with complex psychosocial needs as assigned, promoting a coordinated care model that optimizes performance and best practices.

Responsibilities

• Coordinate patient discharge planning and collaborate with healthcare team members
• Intervene with high-risk or complex psychosocial patients as needed
• Promote a seamless, integrated care process aligned with organizational standards

Qualifications

• Graduate of an accredited Nursing program with a current state license
• Bachelor's Degree in Nursing required
• 2-3 years of experience in an acute care hospital setting preferred
• Care Management or Utilization Management experience preferred
• Certifications: BLS (CPR) required at hire or within 90 days; Case Management certification preferred

Skills

• Strong communication, assessment, time management, critical thinking, and advanced computer skills
• Ability to interact effectively across all organizational levels and adapt care to patient age-specific needs, including neonatal to geriatric
• Knowledge of InterQual or Milliman Care Guidelines is preferred
