Part-Time In-Home Health Clinician

About the Role

We are seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, fostering trusted relationships and improving health outcomes.

Responsibilities

• Conduct 45-minute visits including medication and medical history review, physical assessment, and optional diagnostic tests.
• Utilize an iPad and clinical workflow without prescribing medications or altering treatments.
• Engage in virtual visits and travel within licensed states as needed.
• Provide diagnostic and preventive services such as spirometry and screenings.
• Work primarily in members’ homes, ensuring safety in diverse environments.

Requirements

• Active, unrestricted license(s) and board certification.
• Multi-state licenses preferred.
• Ability to cover specified primary locations.
• Passion for patient engagement, flexibility, and commitment to community health.
• Must be able to safely perform duties in various home environments.
