Part-Time In-Home Clinician

About the Role

This position involves conducting in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members. The clinician will perform pre-scheduled visits, including medical history reviews, physical assessments, and diagnostic tests, without prescribing medication or altering treatment plans.

Responsibilities

• Perform in-home visits within designated metro areas, ensuring a completion rate of 35+ visits per month.
• Maintain active state licenses and board certification, with multi-state licenses preferred.
• Provide a minimum of 12 hours weekly availability, primarily between 8 a.m. and 7 p.m., with flexibility to travel locally.
• Conduct video visits and complete diagnostic and preventive services as trained.
• Ensure proper supplies and diagnostic equipment are maintained and used appropriately.
• Meet operational, clinical, and member experience KPIs and metrics.

Requirements

• Board-certified nurse practitioners with appropriate licensing and certifications (FNP, AGNP preferred).
• Experience with in-home health evaluations or similar settings is preferred.
• Ability to lift up to 20 pounds, sit, stand, and perform physical tasks required for fieldwork.
• Flexible travel within licensed states and field observation experience preferred.
