About the role
We’re seeking part-time In-Home Nurse Practitioners to conduct health evaluations for Medicare, Medicaid, and commercial plan members in their homes.
Responsibilities
- Perform pre-scheduled in-home visits
- Review medical histories
- Conduct physical assessments
- Administer diagnostic tests as ordered
- Maintain high visit completion rates
- Close care gaps through patient engagement
Requirements
- Maintain active state licenses and board certification
- Complete necessary training
- Flexibility to travel locally
- Availability to work between 8 a.m. and 7 p.m.
