Role Overview
This part-time in-home nurse practitioner (NP) conducts health evaluations for Medicare Advantage, Medicaid, and commercial plan members, primarily within local metro areas. Responsibilities include performing scheduled in-home visits, reviewing medical histories, conducting physical assessments, and ordering diagnostic tests when necessary. The role emphasizes engaging with patients in their homes to improve health outcomes, providing education, and closing care gaps, with an emphasis on safety and quality.
Key Duties
- Perform pre-scheduled in-home visits, primarily between 8 a.m. and 7 p.m., with flexibility to work Monday through Sunday
- Complete at least 12 hours weekly, averaging 50+ visits per month and 35+ completed visits
- Maintain clinical documentation and adhere to operational and quality KPIs
- Conduct video visits and maintain relevant training and certifications
- Maintain all required licenses, certifications, and supplies
- Travel locally as needed, up to 25%
Qualifications & Skills
- Board certification as a nurse practitioner (NP), with active state licenses (FNP, AGNP preferred)
- Experience with in-home health evaluations or similar settings (preferred)
- Ability to travel locally, lift up to 20 pounds, and perform physical activities required for fieldwork
- Strong communication and organizational skills, with a commitment to safety and quality care
