Part-Time In-Home Clinician

About the role

This part-time nurse practitioner position involves conducting in-home health evaluations for Medicare, Medicaid, and commercial plan members. The clinician will visit members in their homes within a designated metro area, primarily during weekdays, to review medication and medical history, perform physical assessments, and facilitate diagnostic tests as ordered. Responsibilities include maintaining licenses and certifications, completing scheduled visits, and ensuring high visit completion rates.

Responsibilities

• Conduct pre-scheduled in-home visits, primarily within local metro areas
• Review health histories, perform physical exams, and facilitate diagnostic tests
• Maintain compliance with training, licensing, and certification requirements
• Meet operational KPIs such as visit completion and member engagement
• Support video visits and maintain all necessary supplies and equipment
• Travel as needed within licensed states up to 25%

Qualifications

• Board-certified nurse practitioner (NP), with active state licenses and board certification
• Experience with in-home health evaluations or similar settings (preferred)
• Strong communication, organization, and time management skills
• Ability to lift up to 20 pounds and perform field observations
• Flexibility to work weekdays and weekends, with availability for scheduled visits
