Part Time Home Health Occupational Therapist

Position Overview Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaborate with interdisciplinary teams to optimize patient well-being through assessments of mobility, coordination, and strength, and develop personalized treatment plans. Responsible for documentation, modifying goals, discharge planning, and educating patients and caregivers on home exercises. Supervise personnel such as OT Assistants and Home Health Aides as applicable. Complete assessments like OASIS where permitted by regulation. Requirements Degree in Occupational Therapy from an accredited programUnrestricted state license as an Occupational TherapistValid CPR, driver’s license, and proof of auto insurance Preferred Experience in home health therapy and OASIS documentation Skills And Responsibilities Conduct initial and ongoing evaluations to set functional goalsDeliver therapy services per physician ordersMaintain comprehensive documentation to ensure quality and complianceCoordinate with healthcare team, patients, and familiesParticipate in discharge planning and patient educationServe as a clinical resource and supervise staff as needed
