Part Time Home Health Occupational Therapist

Position Overview Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaborate with a multidisciplinary team to optimize patient well-being through evaluations, treatment plans, and ongoing documentation. Key Responsibilities Perform initial and ongoing assessments of mobility, strength, and functional abilities.Develop and document personalized plans of care with goals, progress, and outcomes.Provide occupational therapy services per physician orders and modify plans as needed.Manage discharge planning, educate patients and caregivers on home exercises.Coordinate with healthcare team members, document accurately, and maintain communication.Supervise relevant personnel and complete assessments where regulated. Qualifications & Skills Degree in Occupational Therapy from an accredited program.Current, unrestricted state license as an Occupational Therapist.Valid CPR, driver's license, and proof of insurance.Preferred: Experience in home health therapy and completing OASIS documentation.
