About the role
We seek an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans.
Responsibilities
- Conduct initial and ongoing assessments of mobility, strength, and functional levels
- Interpret tests and set achievable goals
- Provide written evaluations, develop plans of care, and deliver services aligned with physician orders
- Ensure timely documentation and modifications as needed
- Perform discharge planning
- Provide patient and caregiver education
- Coordinate with healthcare teams
- Supervise personnel such as OT Assistants and Home Health Aides where permitted
- Maintain compliance with documentation policies
- Complete assessments like OASIS within scope
Qualifications
- Degree in Occupational Therapy from an accredited program
- Active, unrestricted state license as an Occupational Therapist
- Valid CPR certification
- Valid driver's license

- Experience in home health therapy
- OASIS documentation experience
