About the Role
Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with an interdisciplinary team, updating patient progress, and providing tailored therapy services.
Responsibilities
- Perform initial and ongoing assessments of mobility, coordination, and function.
- Administer tests, interpret results, and set functional goals and outcomes.
- Prepare written evaluations and care plans outlining therapy needs and progress.
- Deliver occupational therapy services per physician orders and document accordingly.
- Maintain and modify care plans, and develop discharge plans with patient and caregivers.
- Coordinate with healthcare team members, including physicians, nurses, and family.
- Supervise other personnel (e.g., OTA, HHA) as applicable.
- Complete necessary assessments and documentation in compliance with regulations.
Requirements
- Degree in Occupational Therapy from an accredited program.
- Unrestricted state license as an Occupational Therapist.
- Valid CPR certification, driver’s license, and proof of automobile insurance.
Preferred Qualifications
- Experience in home health therapy and OASIS documentation.
