Part Time Home Health Occupational Therapist

About the Role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with an interdisciplinary team, updating patient progress, and providing tailored therapy services.

Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, and function.
• Administer tests, interpret results, and set functional goals and outcomes.
• Prepare written evaluations and care plans outlining therapy needs and progress.
• Deliver occupational therapy services per physician orders and document accordingly.
• Maintain and modify care plans, and develop discharge plans with patient and caregivers.
• Coordinate with healthcare team members, including physicians, nurses, and family.
• Supervise other personnel (e.g., OTA, HHA) as applicable.
• Complete necessary assessments and documentation in compliance with regulations.

Requirements

• Degree in Occupational Therapy from an accredited program.
• Unrestricted state license as an Occupational Therapist.
• Valid CPR certification, driver’s license, and proof of automobile insurance.

Preferred Qualifications

• Experience in home health therapy and OASIS documentation.
