Part Time - Care Manager

Position: RN Care Manager-Weekend Location: Evanston, IL Responsibilities: Facilitate payor authorization, coordinate patient care across the continuum, ensure documentation accuracy, collaborate with multidisciplinary teams, resolve system barriers, monitor patient progress, and intervene to optimize care and discharge processes. Qualifications: Current IL RN license, Bachelor's degree in Nursing (preferred from NLN-accredited school), clinical certification (beneficial), BLS/CPR certification, minimum of 2 years clinical nursing experience, experience in utilization management, managed care, or chronic disease management preferred. Experience with EMR systems and knowledge of InterQual or MCG criteria is a plus. Skills: Strong communication, care coordination, problem-solving, and patient-focused approach.
