About the Role
Plans, organizes, and manages the effective and efficient delivery of Home Care rehabilitation services for the Part B at Home Division. Responsible for development of current and future business ventures and ensuring appropriate staffing and tools to support future expansion and growth.
Qualifications
- Bachelor’s and/or Master’s degree in Physical Therapy, Occupational Therapy, or Speech and Language Pathology
- Current state license in respective therapy discipline
- Minimum of 3+ years of rehabilitation experience
- Proven leadership and program development/supervisory (management) experience
- Knowledge of state, federal, and therapy practice act legislation/law
- Interdisciplinary management and understanding essential
- Excellent oral and written communication skills
- Previous Home Care experience preferred
- Knowledge of clinical compliance policies, practices, standards, and systems
- Demonstrates good judgment and ability to identify methods and techniques for obtaining solutions to complex problems
- Strong leadership, communication, and organizational skills
- Proficient in Oasis Process
Responsibilities
- Collaborates with Part A Home Care and reviews referrals and discharges of Rehabilitation programming to evaluate service delivery and needs
- Assists in recruitment of staff based on potential growth areas
- Manages the day-to-day therapy operations and ensures staff’s compliance with all applicable laws, rules, and regulations, including those related to safety and infection control
- Performs initial evaluations, therapy visits, and therapist field visits as needed
- Facilitates and clears Part B at Home workflows, as required by the EMR system
- Ensures rehab services are initiated timely and works with Regional and Human Resources to ensure sufficient Part B at Home staffing
- Ensures that MD orders are obtained as required by law/regulation, and that changes in condition are appropriately communicated to physicians and other disciplines
- Oversees process to educate therapists regarding preparation and submission of clinical records and reports according to policies, professional standards, and third-party reimbursement standards
- Ensures documentation complies with regulations and treatment plans reflect realistic and achievable goals
- Ensures rehab services provided are medically appropriate for established goals
- Monitors therapist performance to ensure rehab services provided are medically required and appropriate
- Conducts therapy staff training and assesses quality of therapist’s documentation to assure compliance with State and Federal rules and regulations
- Reviews audits and provides corrective action plans and recommendations on any deficiencies
- Handles orientation and training of rehabilitation personnel
- Reviews and updates rehabilitation policy and procedures regularly to ensure compliance with facility processes and state/federal regulations
- Establishes, disseminates, and adheres to HIPAA compliance policies and procedures
- Attends and participates in educational activities, in-service training, and staff meetings; assists in orientation and training of other staff
- Assists in facility and rehabilitation marketing plan, including:
- Referral Rehabilitation Screenings
- Community and referral source in-servicing
- Providing program evaluation and outcome information for inclusion in marketing materials
- Assisting in market analysis and providing recommendations for new rehabilitation programs
- Reviews and makes recommendations concerning staff education needs and requests; provides in-services and obtains speakers to assist staff in improving job performance
- Other duties as assigned by the company

Pay
All rates offered are based on position, years of applicable experience, and employment type. Rate ranges include levels for per diems with no benefits and Independent Contractors.
