Part-Time Care Manager
Position Summary
The Part-Time Care Manager – Social Isolation Grant provides individualized care management services to older adults participating in a grant-funded initiative designed to reduce social isolation and improve overall well-being. This position conducts assessments, develops personalized care plans, connects participants to community resources, and provides ongoing support through home visits and telephone outreach. The Care Manager also ensures compliance with grant requirements through accurate documentation, data collection, and outcome reporting.

Key Responsibilities
- Conduct comprehensive in-home assessments to identify participant needs, strengths, risks, and barriers to engagement.
- Develop, implement, monitor, and update individualized care plans based on participant goals.
- Conduct regular in-home visits and telephone outreach to reduce social isolation and promote participant well-being.
- Maintain accurate, timely, and confidential participant records in accordance with agency policies and grant requirements.
- Connect participants with community resources, healthcare providers, support services, and benefit programs.
- Collaborate with internal staff, healthcare providers, community organizations, and social service agencies to coordinate participant care.
- Perform other duties as assigned to support program and organizational goals.
Qualifications
- Education: Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Gerontology, or a related field preferred.
- Experience: Minimum of two years of experience working with older adults preferred. Case management experience preferred. Experience with community-based or grant-funded programs is a plus.
- Certifications/Licenses: Valid New Jersey driver's license with reliable transportation required. Ability to successfully complete all required background checks.
- Bilingual in English and Spanish required.
