About the role
Reid Health is seeking a patient‑centered and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our PACE (Program of All‑Inclusive Care for the Elderly) team. The APP will provide primary medical management for PACE participants in collaboration with our PACE physicians and interdisciplinary care team, focusing on comprehensive assessments, ongoing care planning, and supporting the health and well‑being of our participants.
Responsibilities
Provide primary medical management for PACE participants, conduct comprehensive assessments, develop and adjust care plans, and work closely with physicians and the interdisciplinary team to ensure coordinated, patient‑focused care.
Schedule
Office Hours: 8 am – 4:30 pm

Requirements
- Successful completion of a qualified NP or PA program
- Indiana professional license
- Current BLS and ACLS certifications
