About the Role

The Occupational Therapist in Home Health evaluates and assesses patients' functional status, activities of daily living, motor coordination, and home environment to develop personalized treatment plans. They provide therapy within legal scope, collaborate with healthcare teams, and consult with physicians.

Responsibilities

• Conduct patient assessments and develop individualized therapy plans
• Evaluate functional abilities and collaborate on treatment strategies
• Monitor and report patient responses and condition changes
• Provide evidence-based occupational therapy services within scope of practice
• Document patient progress and ensure care is delivered regardless of patient background or setting

Requirements

• Current state licensure in Occupational Therapy
• CPR certification
• Valid driver’s license and reliable vehicle or transportation access

Skills

• Strong organizational and communication skills
• Ability to work independently (preferred)
