About the role

Assess and evaluate patients' functional status, daily activities, fine motor skills, and adaptive equipment needs in home health settings, in accordance with state regulations and medical plans.

Responsibilities

• Provide services within the scope of practice, utilizing evidence-based techniques, and collaborate with the healthcare team.
• Evaluate patient function, document CMS requirements, and consult with physicians on care plans.
• Monitor and report patient responses and condition changes to supervisors and physicians.
• Conduct assessments using diagnostic and prognostic tests to determine functional abilities.

Requirements

• Current state licensure in Occupational Therapy.
• CPR certification.
• Valid driver’s license, vehicle insurance, and reliable transportation or access to public transit.
• Ability to work effectively in diverse home environments regardless of client background.
