Position Overview Seeking an Occupational Therapist to provide home health services, including assessment and evaluation of patients' functional status, activities of daily living, fine motor skills, and adaptive equipment needs. The role involves conducting patient assessments, developing care plans in collaboration with physicians, and monitoring patient responses to treatment in various home environments. Key Responsibilities Deliver occupational therapy within legal and professional scope, adhering to evidence-based practices and coordinated team efforts.Assess and document patient conditions, functional abilities, and system statuses for compliance and treatment planning.Communicate patient progress and changes to supervising nurses and physicians.Perform diagnostic assessments to evaluate patient functionality and prognosis. Qualifications & Skills Current Occupational Therapy license in the practicing state.CPR certification and a valid driver's license with reliable transportation.Ability to adapt to diverse home environments and serve clients of all backgrounds.Preferred skills include multitasking, independence, strong communication, organization, and documentation abilities.
