Occupational Therapist - Full Time, Day Shift

About the Role

Provides occupational therapy services within a home health and hospice setting, focusing on restoring and enhancing patients' physical, social, and mental functions. Acts as a co-leader in interdisciplinary teams to develop and implement individualized treatment plans.

Responsibilities

• Conducts evaluations to assess patient needs and develops personalized treatment strategies.
• Delivers therapy interventions using appropriate techniques and protocols.
• Prepares and communicates care and discharge plans, ensuring thorough documentation within deadlines.
• Supervises assigned therapy assistants and performs additional duties as needed.

Qualifications

• Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD).
• Valid occupational therapy license in applicable states.
• CPR/BLS certification and a valid driver's license, with minimum age of 21.
