About the role
We are seeking a qualified Occupational Therapist to join our home health and hospice teams across multiple states.
Responsibilities
- Planning and conducting treatment programs to enhance patients' physical, social, and mental functions
- Serving as a co-leader within an interdisciplinary team
- Performing evaluations, developing individualized care and discharge plans, and ensuring effective communication of treatment protocols
- Supervising assigned assistants and completing accurate clinical documentation
- Performing other related duties as assigned
Qualifications
- Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD)
- Valid licensure in all applicable states
- CPR or BLS certification
- Valid driver's license and age of 21 or older
