Occupational Therapist - Full-Time and PRN

About the Role

Assess and evaluate patients' functional status, daily activities, and occupational therapy needs based on medical conditions. Deliver equitable, quality care and foster professional growth while operating in diverse home environments.

Responsibilities

• Provide services within state practice laws, following the plan of care, using evidence-based techniques, and collaborating with the healthcare team.
• Evaluate patients' functional abilities, coordinate with physicians on therapy plans, and document CMS requirements.
• Monitor, record, and report patient responses and condition changes to supervisors and physicians.
• Conduct assessments using diagnostic and prognostic functional tests.
• Operate effectively in diverse home environments, regardless of patient background.

Requirements

• Current Occupational Therapy licensure in the state of practice.
• Valid driver's license, vehicle insurance, or access to reliable transportation.

Preferred Skills

• CPR certification.
• Ability to multitask and work independently.
• Strong communication, organization, and writing skills.
