About the role
Join a provider of in-home health assessments, conducting comprehensive evaluations for adult and geriatric patients in their residences, including homes and Skilled Nursing Facilities.
Responsibilities
- Performing health assessments covering medical history, diagnosis, treatment, medication adherence, social environment, and risks
- Administering and ordering point-of-care and lab tests as needed
- Collaborating with physicians, case managers, social workers, and caregivers for patient care and follow-up
- Providing patient education and documentation per protocols
Qualifications
- Master’s Degree or equivalent experience with NP licensure and prescription authority
- Board certification in adult or geriatric primary care, family, emergency, or acute care
- Current BLS, ACLS, or CPR certification
- Valid driver’s license and auto insurance, with ability to obtain licensure in multiple states if needed
- Strong communication and computer skills
