Nurse Practitioner or Physician Assistant - PRN

About the role

Join a provider of in-home health assessments, conducting comprehensive evaluations for adult and geriatric patients in their residences, including homes and Skilled Nursing Facilities.

Responsibilities

• Performing health assessments covering medical history, diagnosis, treatment, medication adherence, social environment, and risks
• Administering and ordering point-of-care and lab tests as needed
• Collaborating with physicians, case managers, social workers, and caregivers for patient care and follow-up
• Providing patient education and documentation per protocols

Qualifications

• Master’s Degree or equivalent experience with NP licensure and prescription authority
• Board certification in adult or geriatric primary care, family, emergency, or acute care
• Current BLS, ACLS, or CPR certification
• Valid driver’s license and auto insurance, with ability to obtain licensure in multiple states if needed
• Strong communication and computer skills
