Responsibilities

• Conduct adult/geriatric health assessments in members’ homes or facilities.
• Review medical history, medication adherence, and social determinants of health.
• Collaborate with healthcare professionals to improve care and identify undiagnosed conditions.
• Administer and order diagnostic testing as needed.
• Document findings and facilitate referrals for treatment and case management.

Requirements

• Master’s Degree or equivalent in Nursing.
• Current Nurse Practitioner licensure with prescriptive authority.
• Board certification in a relevant specialty (e.g., Family, Adult, Gerontology).
• Valid driver’s license and auto insurance coverage.
• Strong communication and computer skills.
