Nurse Practitioner or Physician Assistant - PRN

Role Overview

This position involves conducting comprehensive in-home health assessments for adult and geriatric patients, reviewing medical history, medication adherence, social factors, and other health risks to improve care quality and identify undiagnosed conditions.

Responsibilities

• Perform assessments lasting up to one hour at members' residences, including health education, diagnosis, treatment, referrals, and documentation.
• Administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers for coordinated care.
• Support patient education and follow-up, working in various settings including homes and facilities.

Qualifications & Skills

• Master's Degree or equivalent, with current NP licensure and prescriptive authority.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care.
• CPR/BLS/ACLS certifications, valid driver's license, and insurance coverage.
• Strong communication skills and computer literacy.
• Preferred: 1+ year NP experience; new grads encouraged.
