Nurse Practitioner or Physician Assistant - PRN

Role Overview This position involves conducting comprehensive in-home health assessments for adult and geriatric patients, reviewing medical history, medication adherence, social factors, and other health risks to improve care quality and identify undiagnosed conditions. Responsibilities Perform assessments lasting up to one hour at members' residences, including health education, diagnosis, treatment, referrals, and documentation.Administer point-of-care and lab testing as needed.Collaborate with physicians, case managers, social workers, and caregivers for coordinated care.Support patient education and follow-up, working in various settings including homes and facilities. Qualifications & Skills Master’s Degree or equivalent, with current NP licensure and prescriptive authority.Board certification in Adult, Gerontology, Family, Emergency, or Acute Care.CPR/BLS/ACLS certifications, valid driver’s license, and insurance coverage.Strong communication skills and computer literacy.Preferred: 1+ year NP experience; new grads encouraged.
