Nurse Practitioner or Physician Assistant - PRN

Role Overview

Provide comprehensive in‑home health assessments for adult and geriatric patients, evaluating medical history, medication adherence, social factors, and health risks during visits up to one hour. Collaborate with healthcare professionals, administer tests, and document findings to improve patient care and close care gaps.

Responsibilities

• Conduct assessments, provide health education, and make referrals.
• Order and administer point‑of‑care and laboratory tests.
• Work with physicians, case managers, social workers, and caregivers.
• Follow documentation protocols and collaborate on patient follow‑up.
• Provide services at patient residences, including homes and skilled nursing facilities.

Qualifications & Skills

• Master’s degree or equivalent experience with NP or PA licensure.
• Current licensure with prescriptive authority; board certification in Adult, Gerontology, Family, or Emergency Care.
• CPR/BLS/ACLS certification; at least one year of clinical experience preferred.
• Valid driver’s license, automobile insurance, strong computer skills, and excellent communication abilities.
