Nurse Practitioner or Physician Assistant - PRN

About the Role

The provider conducts comprehensive in-home health assessments, reviewing members' health, medical history, medication adherence, and social factors to improve care quality and identify undiagnosed conditions.

Responsibilities

• Perform adult/geriatric assessments including medical history, diagnosis, treatment, and health education.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, family, and caregivers.
• Coordinate follow-up and provide patient education in members' homes or facilities.

Requirements

• Master’s Degree or equivalent experience with NP licensure; current state licensure with prescriptive authority.
• Board certification in relevant adult or family practice specialties.
• Current BLS, ACLS, or CPR certification.
• Minimum one year NP experience preferred; new graduates encouraged.
• Valid driver’s license, auto insurance, and computer proficiency.
• Excellent communication skills.
