Nurse Practitioner or Physician Assistant - PRN

About the role

Seeking a Nurse Practitioner to perform comprehensive in-home health assessments for adult and geriatric patients, focusing on medical history, medication adherence, social factors, and health risks.

Responsibilities

• Conduct detailed assessments, including medical history, diagnosis, treatment, and health education.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and caregivers.
• Coordinate follow-ups and referrals to ensure continuity of care.
• Provide services at members' residences, including homes and Skilled Nursing Facilities.

Qualifications

• Master’s Degree or equivalent with NP licensure and prescriptive authority.
• Board certification in relevant adult or family care specialties.
• Current BLS, ACLS, or CPR certification.
• Valid driver’s license and automobile insurance.
• Strong computer skills and excellent communication abilities.
