About the Role
This position involves conducting comprehensive in-home health assessments for patients, including reviewing medical history, medication adherence, social factors, and risks. The provider collaborates with healthcare professionals, administers tests, and provides patient education and follow-up care, primarily at patients' residences.
Responsibilities
- Perform adult and geriatric health assessments, documenting findings per protocols
- Administer and order point-of-care and lab tests
- Coordinate with physicians, case managers, social workers, and caregivers
- Support patient education and follow-up in coordination with primary care providers
- Deliver services in patients' homes or skilled nursing facilities
Requirements
- Master’s Degree or equivalent experience with NP licensure and prescriptive authority
- Board certification in relevant adult or family care specialties
- Current BLS, ACLS, or CPR certification
- At least one year of NP experience preferred; new graduates encouraged
- Valid driver’s license and automobile insurance
