Nurse Practitioner or Physician Assistant - PRN

About the Role

This position involves conducting comprehensive in-home health assessments for patients, including reviewing medical history, medication adherence, social factors, and risks. The provider collaborates with healthcare professionals, administers tests, and provides patient education and follow-up care, primarily at patients' residences.

Responsibilities

• Perform adult and geriatric health assessments, documenting findings per protocols
• Administer and order point-of-care and lab tests
• Coordinate with physicians, case managers, social workers, and caregivers
• Support patient education and follow-up in coordination with primary care providers
• Deliver services in patients' homes or skilled nursing facilities

Requirements

• Master’s Degree or equivalent experience with NP licensure and prescriptive authority
• Board certification in relevant adult or family care specialties
• Current BLS, ACLS, or CPR certification
• At least one year of NP experience preferred; new graduates encouraged
• Valid driver’s license and automobile insurance
