Nurse Practitioner or Physician Assistant - Full Time Travel Role

Responsibilities

Conduct comprehensive in-home health assessments for adult and geriatric patients, including reviewing medical histories, medication adherence, social factors, and performing point-of-care testing, documenting findings, and collaborating with physicians, case managers, and caregivers to improve care quality and address care gaps.

Qualifications

• Master’s Degree or equivalent with NP/PA licensure and prescriptive authority
• Board certification in Adult, Gerontology, Family, or Emergency Care
• Current BLS, ACLS or CPR certification
• At least 1 year of NP or PA experience
• Ability to travel extensively, with valid driver’s license and insurance
• Strong communication and computer skills
