Nurse Practitioner or Physician Assistant - Full Time State Traveler

About the role

We are seeking a healthcare professional to conduct comprehensive in-home assessments for adult and geriatric patients, reviewing medical history, medication adherence, social factors, and health risks to improve care quality and close care gaps.

Responsibilities

• Performing assessments, documenting findings, and providing health education
• Administering or ordering point-of-care and lab testing
• Collaborating with physicians, case managers, social workers, and caregivers
• Referring patients for further treatment and follow-up
• Providing services at members' residences, including homes and Skilled Nursing Facilities

Qualifications and Skills

• Master’s Degree or equivalent experience with NP/PA licensure and prescriptive authority
• Board certification in relevant specialties (Adult, Gerontology, Family, Emergency, or Acute Care)
• Current BLS/ACLS/CPR certification
• At least 1 year of NP or PA experience
• Ability to travel extensively, with a valid driver’s license and insurance
• Strong communication and computer skills
