Nurse Practitioner or Physician Assistant - Full Time State Traveler

About the role

Join a national provider delivering in-home health assessments to improve patient outcomes and care quality. As a provider, you will conduct comprehensive assessments lasting up to one hour, reviewing medical history, medication adherence, social factors, and health risks.

Responsibilities

• Perform adult/geriatric evaluations in members’ residences, including homes and facilities
• Administer tests and review medical history, medication adherence, social factors, and health risks
• Collaborate with healthcare teams to support patient care plans
• Provide education and follow-up to patients

Requirements

• Master’s Degree or equivalent experience
• Current NP/PA licensure with prescriptive authority
• Board certification in relevant specialties
• At least one year of clinical experience
• Valid driver’s license and appropriate insurance coverage
• Ability to travel extensively
