About the role
Join a national provider delivering in-home health assessments to improve patient outcomes and care quality. As a provider, you will conduct comprehensive assessments lasting up to one hour, reviewing medical history, medication adherence, social factors, and health risks.
Responsibilities
- Perform adult/geriatric evaluations in members’ residences, including homes and facilities
- Administer tests and review medical history, medication adherence, social factors, and health risks
- Collaborate with healthcare teams to support patient care plans
- Provide education and follow-up to patients
Requirements
- Master’s Degree or equivalent experience
- Current NP/PA licensure with prescriptive authority
- Board certification in relevant specialties
- At least one year of clinical experience
- Valid driver’s license and appropriate insurance coverage
- Ability to travel extensively
