Nurse Practitioner or Physician Assistant
About the role
Practice medicine in a way that prioritizes time, connection, and meaningful patient relationships. As a Nurse Practitioner or Physician Assistant in Physician Services, you'll provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on what matters most—delivering high-quality, patient-centered care that improves comfort, function, and quality of life.
In this role, you'll conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. Working within a collaborative practice framework, you'll serve as a key clinical decision-maker while helping patients and families better understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations.
Responsibilities
- Conduct comprehensive medical evaluations
- Develop and manage individualized treatment plans
- Prescribe and monitor medications
- Care for patients in their homes, assisted living facilities, or nursing homes
- Coordinate with physicians, caregivers, and interdisciplinary team members
- Serve as a key clinical decision-maker
- Help patients and families better understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations
Requirements
- Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant
- Current State License as a Nurse Practitioner or Physician Assistant
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
Qualifications
- Minimum 2 years clinical experience in a post-acute or as a Nurse Practitioner or Physician Assistant
- Adult, Family, or Gerontology certification
- Experience with geriatric patients
Skills
- Nursing skills as defined by accepted nursing standards
- Ability to assess and respond to the needs of patients, families and caregivers in post-acute care settings
- Ability to cope with family emotional stress and tolerance of individual lifestyles
- Handle multiple priorities, documentation requirements and deadlines

Pay
This role provides a base annual salary (listed below) and has monthly bonus potential.
Pay Range $110,000—$125,000 USD
Schedule
Monday to Friday, 8 a.m. to 5 p.m.
