Nurse Practitioner or Physician Assistant
Atlas Medical is expanding our Home Health & Hospice Provider Team and seeking a dynamic, experienced Nurse Practitioner or Physician Assistant who shares our mission to make healthcare better for everyone.
About the role
As a Medical Provider for our Atlas Medical Team, you will deliver mobile on-site primary care services to vulnerable patients in assisted living and other senior communities. This role requires autonomous management of complex healthcare scenarios, including multiple co-morbid conditions. Beyond medical treatment, you will educate patients and families on self-care, disease management, and end-of-life care. You will collaborate daily with facility staff and Atlas Patient Care support teams, guided by quality measures to uphold high standards of care. Responsibilities
- Provide mobile on-site primary care to patients in assisted living and senior communities
- Manage complex healthcare scenarios, including multiple co-morbid conditions
- Educate patients and families on self-care, disease management, and end-of-life care
- Collaborate with facility staff and Atlas Patient Care support teams
- Adhere to quality measures and maintain confidentiality
Requirements
- At least one (1) year of primary care experience
- Valid licensure as a Nurse Practitioner or Physician Assistant in the state of practice
- Ability and desire to work independently and drive between communities
- Experience with Elation EMR
- Effective verbal and written communication and time management skills
- Valid AZ Fingerprint Clearance Card
- Current CPR certification and proof of negative TB test

Pay
$70 per hour for up to 6 months, then transitioning to fee-for-service (FFS).
Schedule
Contract position with full-time hours.
