Nurse Practitioner (NP) Primary Care

About the role

Manage chronic conditions of your own patient panel from Monday to Friday, with no weekends or evenings.

Responsibilities

• Nurse Practitioners with direct experience in home health, geriatrics, hospice, and primary care.
• A focus on patient care rather than volume.
• To take the time needed to address the patient's needs.

Requirements

• Licensed as a NP in the State where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Maintain a valid driver’s license and good driving record.

Qualifications

• Experience in managing chronic conditions of patients in home settings.
• Strong geriatric care skills.
• Effective communication and interpersonal skills.
• Ability to manage a caseload and provide comprehensive care.

Skills

• Proficiency in using Athena EMR and AI Scribe for charting.
• Excellent clinical judgment and problem-solving skills.
• Strong organizational and time management skills.
• Ability to work independently and as part of a team.
