About the role
Manage chronic conditions for 6-8 patients per day, Monday to Friday, from 8 AM to 4:30 PM.
Responsibilities
- Directly related experience in home health, geriatrics, hospice, and primary care
- Focus on patient care rather than volume
- Take time to address patient needs
Requirements
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Geriatric training/experience preferred
- Active CPR/BLS Certification
- Board Certification
- Maintain a valid driver’s license and good driving record
Qualifications
