About the role

Manage chronic conditions of your own patient panel from Monday to Friday, with no weekends or evenings.

Responsibilities

• Manage a patient panel with chronic conditions
• Chart using Athena EMR + AI Scribe
• Practice medicine with a focus on patient care, not volume
• Take the time needed to address patient needs

Requirements

• Licensed as a NP in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Maintain a valid driver’s license and good driving record

Qualifications

• Directly related experience in home health, geriatrics, hospice and primary care
