About the role
Manage chronic conditions of your own patient panel from Monday to Friday, excluding weekends and evenings. Use Athena EMR + AI Scribe for charting during workday.
Responsibilities
- Nurse Practitioner with direct experience in home health, geriatrics, hospice, and primary care
- Focus on patient care rather than volume
- Take time to address patient needs thoroughly
Qualifications
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Geriatric training/experience preferred
- Active CPR/BLS Certification
- Board Certification
- Maintain a valid driver’s license and good driving record
