About the Role
The Nurse-Family Partnership Nurse Home Visitor provides comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors play a pivotal role in transforming the lives of vulnerable babies, mothers, and families by implementing and maintaining fidelity to the model.
This is a community-based position where you will visit parents in their home or a community location, carrying a caseload of 25-30 clients. The role requires travel within New Castle County for family visits, training, and meetings, as well as flexible hours, including nights and weekends as needed.
Responsibilities
- Share resources, perform health checks, and partner with first-time expectant parents during pregnancy and until the child is two years old.
- Provide support to parents to uncover their innate strengths and skills as parents.
- Complete extensive model training by the NFP National Service Office and participate in ongoing reflective supervision with site supervisors.
- Adhere to company and program guidelines.
Requirements
- Registered Nurse with a minimum of an Associate's degree in Nursing and a current RN license in good standing.
- Experience in public health, community, and/or maternal-child health (required).
- Experience with labor and delivery (desirable).
- Home visiting experience (preferred).
- Bi-lingual (English/Spanish) (desirable).
- Current CPR certification.
- Valid driver’s license that meets the agency’s insurance carrier requirements.

- Proficiency with computers, including Microsoft Office.
